Legal Classification of medication

POM

Donepezil tablets or injection can only be obtained in accordance with a prescription given by an appropriate practitioner and therefore the legal status of the medication is prescription only medication (POM).

Formulations

Donepezil is available in 2 strengths and comes as tablets and orodispersible tablets.

  • Film coated tablets 5mg and 10mg

Class of drug

Donepezil is a specific and reversible inhibitor of acetylcholinesterase.

Indications

Donepezil is indicated for treatment mild to moderately severe dementia.

How does it work?

Donepezil works by blocking the effects of an enzyme in the brain called acetylcholinesterase. By doing this, this increases the levels of acetylecholine, an important neurotransmitter in communication between neurons (nerve cells in the brain).

How to take it?

The initial starting dose is 5mg, taken in the evening before bedtime. This dose should be continued for 1 month (time taken for the drug to reach steady state and enough time to assess clinical response) and then the patient should be reassessed. If necessary the dose may then be increased to 10mg. The orodispersible tablets should be placed on the tongue, allowed to disperse, and swallowed.

It is important that treatment only be commenced if a caregiver is available who will regularly monitor compliance with the treatment regime. Treatment may be continued so long as the patient continues to benefit.

Pregnancy and Breast feeding

The effects of donepezil on pregnancy and breastfeeding are unknown in humans and therefore should not be given.

Contra-indications

Donepezil should be avoided if the patient has an allergy to piperidine derivatives.

Side-effects

Side-effects of donepezil include:

  • Diarrhoea
  • Nausea
  • Headache
  • Agitation
  • Hallucination
  • Blackouts
  • Rash
  • Itching
  • Muscle cramps
  • Urinary incontinence

Side effects which are uncommon and rare include:

  • Seizure
  • Slow heart rate
  • Stomach ulcers
  • Bleeding from stomach or bowel
